Op 8 oktober 2009 is dr. Jasper Reenalda gepromoveerd aan de Universiteit Twente. In zijn proefschrift "Dynamic sitting to prevent pressure ulcers in spinal cord injured"  wordt het risco op decubitus belicht aan de hand van het in kaart brengen van risicofactoren en de invloed van dynamisch zitten in relatie tot het ontstaan en voorkomen van decubitus.

Summary:

At present, clinical interventions and research efforts are not fully successful in defining the risk of pressure ulcer development and as such in eventually preventing pressure ulcers. As a result, the prevalence and incidence values of pressure ulcers remain unacceptably high. It is common sense that healthy subjects do not develop pressure ulcers due to a dynamic variation in sitting posture. This dynamic sitting behavior can be identified and used as a reference values for the optimal sitting behavior of individuals that use a wheelchair and are at risk of developing pressure ulcer. The main research question that has been answered in this thesis is if imposing dynamic sitting behavior by means of the Dynasit chair in Spinal Cord Injured individuals results in elevated tissue viability? In order to define a reference values for the sitting behavior of spinal cord injured individuals, an analysis of healthy sitting behavior has been performed. It was found that healthy subjects change their sitting posture at least every eight minutes. To impose this dynamic sitting behavior on spinal cord injured individuals, the Dynasit chair has been developed. The Dynasit chair is an experimental sitting device developed to investigate the effects of different sitting postures, alternating tuberal support and pelvic orientations on the (sub-) cutaneous tissue viability. Two studies on healthy subjects demonstrated the positive effect of actuation with the Dynasit chair on the tissue load. Regarding the main research question of this thesis, a study performed in 10 male spinal cord injured individuals, confirmed the ability of the Dynasit chair to impose dynamic sitting behavior, based on healthy sitting behavior, in spinal cord injured individuals. This imposed dynamic sitting behavior resulted in significant elevation of the (sub-) cutaneous tissue oxygenation. This elevated tissue oxygenation implies that due to the dynamic sitting, the wheelchair user can sit for prolonged periods of time without tissue breakdown.

Summary

 

Many wheelchair-users with reduced postural stability adopt a static body posture and have problems with the performance of functional movement tasks. Prolonged static sittingwith a flexed spinal posture and posterior tilted pelvis unconditionally leads to all kinds of physical discomfort including the development of pressure ulcers and low back injury. Most of these problems occur from sustained mechanical tissue loading. Dynamic seating interventions are therefore needed to alleviate the load bearing tissue periodically. We developed an experimental simulator chair and evaluated several seating interventions that are designed to enhance functional movement and to prevent physical discomfort due to prolonged static sitting. Theoretical andexperimental evaluations were performed in able-bodied subjects and in subjects with a spinal cord injury (SCI). The following research questions have been answered:

 

How can sitting posture be controlled?

 

Based on a parallelogram design that aligns the chair pivots with the anatomical axes for body segments rotation, we developed a technique that adjusts all three body segments (trunk, pelvis, thighs) separately.Together with simultaneous measurement of body segments orientation, so-called body segments decoupling seemed applicable to control sitting posture when individuals lack the strength to do this themselves.

 

What interventions are effective to regulate body load associated with physical discomfort (i.e. pressure ulcers and low back pain)?

 

A combination of seat inclination, sagittal decoupled pelvis rotation and a dynamic tuberal support appeared beneficial to regulate the load bearing tissue of the ischial buttock region and lower back simultaneously.

 

 

What interventions benefit the performance of functional movement in impaired sitting?

 

It has been suggested in literature that seating interventions which adjust pelvis angle in sagittal direction might influence spinal posture and the performance of functional movement.We investigated this in ten male SCI-individuals and found that decoupled pelvis alignment affects balance control in impaired sitting and that anterior tilted pelvis postures potentially benefit the functional performance in daily wheelchair-use. 

 

Medio 2007

 

Medio 2007 is het boek 'Zo zit het!' van dr.ir. H.A.M. Staarink verschenen. Hieronder staat de tekst op de achterflap weergegeven.

De meeste Nederlanders brengen eenderde van hun leven zittend door. Goed zitten is een kunst. De keuze voor een goede zithouding is minder ingewikkeld en minder persoonlijk dan iedereen denkt. In feite ligt de uitkomst al sinds 1969 vast in het congresverslag 'Sitting Posture'. Alleen de uitleg daarbij ontbrak tot nu toe. Door een verbeterde definitie van zithouding en uitgebreid onderzoek door de auteur kunnen de uitkomsten van bijna veertig jaar geleden nu eindelijk begrepen en toegepast worden, en dat gebeurt in 'Zo zit het!'.

Bij de analyse van het zitten zijn inspanning en ontspanning sleutelwoorden. Zodra er daarvoor gelegenheid is, zoekt de mens een houding die zo min mogelijk energie kost. Actie vraagt om het tegenovergestelde, om een actieve zithouding, om inspanning. De houding die bij een bepaalde activiteit hoort, is - behalve van stoeleigenschappen - vooral afhankelijk van het zitgedrag: het zich bewust zijn van wat er lichamelijk gezien gebeurt tijdens het zitten en op basis daarvan de juiste keuzes maken.

Een goede zithouding is een investering in de toekomst en voorkomt complicaties en klachten. De bevindingen die de auteur in "Zo zit het!' beschrijft, hebben verstrekkende gevolgen voor vrijwel alle typen zitmeubelen, van kantoorstoel tot vliegtuigstoel, maar zijn met name van belang voor diegene die rolstoelen ontwerpt, fabriceert en - niet op de laatste plaats - voor de rolstoelgebruiker zelf en zijn (professionele) begeleider.
